How ClaimConnect works
With Aviva's ClaimConnect app, the policyholders of our corporate plans can find the nearest panel clinic, file claims, check claim status and manage policies all in one platform anywhere anytime.
Important note: Depending on the type of coverage taken up, the functions available on the app are variable.
Follow the steps below to make claims for your corporate plans
Submit your claim online
Click on Claims / Make a claim to begin your claim submission.
When submitting, there are certain documents (medical bills, reports, or referral letters, etc.) that will be required. Always keep your original documents in order for your claim to be dealt with quickly.

Visit the form library below and complete the relevant documents. You may submit the form via app or portal.
We'll review your claim
We will process your claims within 10 to 14 working days after receiving all the required documents. For further clarification, we will get in touch.

How to make a commercial claim
The following guidelines are for commercial claims - Property All Risks, Work Injury Compensation and Public Liability claims.
Contact us or your intermediary
If the incident requires our immediate attention, please contact your intermediary or us at 68279966.
Take reasonable steps to minimise further loss or damage.
Please give your full support to the loss adjuster appointed by us.
Download the claim form and collate supporting documents
Download and complete the claim form.
The suggested supporting documents are stated in the respective claims forms.
Make a police report pertaining to the incident if necessary.
Take photographs of the subject items and retain the items for our assessment.
Return to us by email
Email the completed claim form and relevant documents to us.
