Eligibility Criteria

  • Staff/members who are between 16 and 70 years old (Age Next Birthday) of Anytime Fitness;
  • Staff/members of Anytime Fitness who have completed the vaccination process. The vaccine must be authorised by the Health Sciences Authority or brought in under Special Access Route (SAR), and you're medically eligible to receive it; 
  • Singaporean citizen or Singapore Permanent Resident with a valid NRIC or FIN. FIN includes Employment Pass / S Pass / Dependant Pass / Student Pass / Long Term Visit Pass / Work Permit (exclude foreign domestic helper or construction worker); and 
  • Residing in Singapore (not out of Singapore for more than 90 continuous days during the coverage period).

3.      I am currently residing in (not out of Singapore for more than 90 continuous days during the coverage period).

4.      I am between 16 to 70 years old at the time of application.

5.      I have completed the Full Vaccination Regimen. The vaccination received has been authorized by the Health Sciences Authority or brought in under Special Access Route (SAR), and I’m medically eligible to have received it.

6.      This application for insurance has been completed wholly by me and not by any other person. I understand that all benefits will be payable only upon an occurrence of an insured event.

7.      I understand that I may seek advice from a financial advisor representative before making a commitment; and in the event that I do not seek advice from a financial advisor representative, I should consider whether the insurance coverage is suitable for my needs.

8.      I acknowledge that I have access to a copy of Product Summary for this complimentary insurance and I have read and understood the content, I am aware of and agree to abide by the Policy terms, conditions and exclusions.

9.      I understand that the insurance coverage is for 12 months, and will commence 14 calendar days from the date this application form is submitted to Aviva and upon the issuance of the Certificate of Insurance.

10.   I acknowledge that Aviva will not be legally responsible for any claims until Aviva has accepted this application.

11.   I understand that I am entitled to a maximum of 1 complimentary COVID-19 Safe Pass coverage from Aviva.

12.   I understand that the policy does not cover any claims if I’m diagnosed with COVID-19 during the waiting period before the policy commences.

13.   I understand that the policy does not cover any loss, damage or liability directly or indirectly caused or arising as a result of any pre-existing medical condition.
