In an unfortunate event that I pass away, how will the death benefit be paid out?
In the event of a Death Benefit, Aviva will pay the first S$150,000 to the proper claimant. The balance of the Death Benefit will be paid according to the prevailing insurance laws and other relevant laws pertaining to estates of deceased. The Nomination of Beneficiary framework under the Insurance Act is not applicable for Group policy arrangement.
Can I claim for hospitalization expenses under Group Term Life policy?
Group Term Life pays in the event of Death, Total Permanent Disablement or Advance Payment Benefit but not for hospitalization expenses. However, this insurance does pay daily hospital cash benefit, for continuous hospital confinement starting from the 11th day of the hospitalization (up to a maximum of 30 days per hospital stay).
If I am insured with Group Term Life policy with Living Care policy, and I am diagnosed with one of the 37 critical illnesses, what will happen to my Group Term Life policy?
If you are diagnosed with one of the 37 critical illnesses and the claim is admitted and paid by Aviva Ltd, the sum assured of your Group Term Life policy will remain unchanged. You may continue with your Group Term Life policy at the prevailing monthly premium rate.
What shall I do if I do not have enough money in my bank account for the monthly GIRO deduction?
You may wish to visit our Customer Service Hall at 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 to pay your premium using cash/NETS/cheque before the last business day of the month.
How can I make my monthly premium payment if I have insufficient funds in my GIRO account and am currently not in Singapore?
You may wish to make arrangement with your family members in Singapore to make the payment. They can visit our Customer Service Hall at 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 to pay the premium using cash/NETS/cheque before the last business day of the month.
How do I change my GIRO bank account?
Please complete a new GIRO application form and mail the form back to Aviva Ltd. Please note that you should not terminate your existing GIRO bank account until your new GIRO bank account is approved.
If you have terminated your existing bank account, please submit a new GIRO application form together with 3 months' premium payment via cheque made payable to 'Aviva Ltd'. On the reverse side of the cheque, please indicate your policy number (s), Client Reference Number, contact number and "Payment for MINDEF & MHA Group Insurance".
However, if you are changing your GIRO bank account between POSB Bank and DBS Bank, you can simply do so by visiting any of the bank branches and the change can be effected immediately. The Bank will keep us informed of the change.
When will the premiums be deducted from my GIRO account?
GIRO deduction is done on the 10th of every month. If the deduction is unsuccessful, a 2nd attempt will be done on the 25th of the same month. If the deduction dates fall on a weekend or public holiday, the banks will bring it forward to an earlier working day.
What is the grace period for payment of premium?
The grace period begins on the premium due date and lasts for 30 days.
Is there a free look period?
Within fourteen (14) days upon receipt of the Policy, you may write to us to cancel the Policy. After we have received the original Policy for cancellation, we will refund the premium you paid (without interest) after deducting any expenses incurred in issuing the policy and any amount of claim that were paid out. If the policy is sent to you by post, you are considered to have received it seven (07) days after the postage.
Will I receive annual renewal notice?
There is no annual renewal notice or renewal letter. The policy year starts on 1 July of the current year to 30 June of the following year for MINDEF/SAF insured persons; 1 July of the current year to 30 June of the following year for MHA insured persons. Renewal is automatic every year, all the way up to age 70 (ANB). However, once a year you will receive a statement from Aviva Ltd on your insurance coverage and the premiums you had paid for the preceding year.
Insured members/insured affiliate members who pay premium yearly will receive a billing notification, one month before the expiry of the policy year.
Will a new policy document be given to me upon renewal every year?
No. Renewal is automatic up to age 70 (ANB). You will be issued with a simple insurance certificate when you first sign up. However, once a year at the close of every calendar year, Aviva Ltd will mail to all insured members/insured affiliate members a statement on their insurance coverage and the premiums they had paid in the preceding calendar year. The information in the statement is also to facilitate your filing for income tax reporting.
Once the premium is paid, does Aviva issue a receipt?
For insured persons who pay cash at the Customer Service Hall, a receipt will be issued. No receipt is offered for those who paid by cheque.
Can the premiums paid be used for tax relief?
Yes, the premiums can be used for income tax reporting, subject to the conditions set by the Inland Revenue Authority of Singapore (IRAS). Aviva will send a consolidated premium statement in the beginning of each calendar year to all insured members/insured affiliate members for premiums paid in the preceding calendar year, for tax reporting.
How do I terminate my/dependant's MINDEF & MHA Group Insurance policy?
To terminate, you will need to submit a signed hardcopy letter and make a photocopy of your NRIC for authentication. Please send it to: Aviva Ltd 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 (Attention: Group Life and Health Services). Alternatively, you can scan the documents and send via email to: MINDEF_insurance@aviva-asia.com (for MINDEF/SAF personnel) or MHA_insurance@aviva-asia.com (for MHA personnel). Upon receipt, we will terminate it on the last day of the month.
Can I come to your Customer Service Hall to sign up for MINDEF & MHA Group Insurance Voluntary Scheme or to terminate this insurance?
Yes, visit Aviva Ltd 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 and our staff will be able to assist you. Our opening hours are: Mondays to Fridays from 8.45am - 5.30pm. We are closed on Saturdays, Sundays and Public Holidays.
Why is it that I am not given an insurance policy document? I was only given an insurance certificate many years' back, but I have lost it. What is the procedure to request for duplicate policy or insurance certificate?
We do not issue policy document to the individuals. The insured members/insured affiliate members of the group scheme are issued with a simplified insurance certificate. This certificate is only issued once. However, if you would like to request for a reprint of the Certificate of Insurance, we will be able to assist. Change of Personal Particulars
I have formally changed my name. Please advise me how I can update my insurance record?
How do I update my address for my MINDEF & MHA Group Insurance?
I have received a lapse/termination letter, which stated that there were insufficient funds in my bank account last month. Can you advise me how I can reinstate my lapsed policy?
To reinstate the lapsed policy, you will need to complete and sign on a Reinstatement Form. Together with the Form, you will need to pay for the premiums in arrears plus an advance of 3 months' premium (by Cheque payment). Please submit a completed GIRO form, so that we can assist you to activate a new GIRO account for the purpose of monthly premium. However, if you have not changed or terminated your bank GIRO account, we can continue to deduct the premiums from your current GIRO account. However, reinstatement of your lapsed policy is subject to your current medical condition(s) which must be acceptable to our underwriters and you will be informed officially in writing.

Will my pre-existing condition be excluded?
Pre-existing condition(s) if not declared when he/she first applied or subsequently apply to upgrade his/her coverage will not be honored in the event of a claim. Pre-existing condition(s) will be subjected to underwriting and you will be communicated in writing of such underwriting decisions.
How long will it take to approve my application for MINDEF & MHA Group Insurance policy and commence coverage?
Your coverage will commence within 5 working days upon receipt of an application if the health declaration is acceptable by the underwriters. However, if there is a medical issue and further medical evidence(s) is/are required, it would take longer, depending on when the applicant can furnish the evidence to support his application.
Nomination of Beneficiaries
Can I make nomination of beneficiaries for MINDEF & MHA Group Insurance Voluntary Scheme?
The Nomination of Beneficiaries framework is not applicable for group insurance scheme. The main policyholder, that is, MINDEF and MHA, had directed Aviva to pay according to the prevailing laws of Singapore. That is to say, in the event of a Death Claim, the law provides for Aviva to pay to the immediate next-of-kin (NOK) or proper claimant up to S$150,000. The payout of the balance of the insurance compensation has to be decided by a Court.
If the deceased had written a Will before his/her death, the family has to engage a lawyer to apply to the Court for a Probate, which may take up to 6 months.
If the deceased did not have a Will before his/her death, he/she would be considered to have died intestate. In such an event the family also has to engage a lawyer to apply to the Court for a Letter of Administration, which may take up to 3 years to resolve.
