Eligibility and Application
Who are eligible for MINDEF & MHA Group Insurance Voluntary Scheme?
- Full-Time National Servicemen (NSFs)
- Operationally Ready National Servicemen (NSmen)
- Regulars (Full-time, Part-time and Contract Service) servicemen in the:
- MINDEF / MINDEF and MHA [including Defence Executive Officers (DXOs)]
- Singapore Police Force (SPF)
- Singapore Civil Defence Force (SCDF)
- Singapore Prison Service (SPS)
- Central Narcotics Bureau (CNB)
- Immigration & Checkpoints Authority (ICA)
- Public Officers working with MINDEF or MHA
- MINDEF and MHA Volunteer Corps (MINDEF and MHA VC)
- NS Volunteers (or equivalent)
- Reservist On Voluntary Extended Reserve Service (ROVERS)
- SPF Volunteer Special Constabulary (VSC)
- SCDF Civil Defence Auxiliary Unit (CDAU)
- Employee (Full-time, Part-time and Contract Service) of the Authority-affiliated entities including:
- Defence Science & Technology Agency (DSTA)/DSTA-affiliated entity
- MINDEF-related Organisations (MROs)
- MHA-related Organisations
The entry age must not be above age 65.

Please enclose a cheque for 3 months' advance premiums so that we can process your application while waiting for your GIRO to be approved.
If you are eligible to apply and are applying for your dependants, you will need to complete one application form for each insured person. Only one GIRO form is required per family. Your spouse and/or your child can apply with you.
I had recently done a full medical examination. Can I use the report for the purpose of applying for this insurance?
For the purpose of applying for the insurance, we only require you to declare your health condition in the Health Questionnaire. For some, we may require you to undergo specific medical tests or checks.
You may also submit a photocopy of your recent medical examination report with your application form. However, the check-up must be within 6 months from your submission date. If your medical report is inadequate or not relevant, our insurance underwriters will communicate with you through phone or mail.
Can I insure my children?
Yes. Your children from birth are eligible to take up MINDEF & MHA Group Insurance provided you are eligible to apply as the main insured person or have an existing MINDEF & MHA Group Term Life or/and Group Personal Accident policy. Your children must be below the age of 25 at next birthday and are unmarried.
What are the policies offered under the MINDEF & MHA Group Insurance Voluntary Scheme?
There are 5 policies available. These are the Group Term Life (main policy) with maximum coverage of S$1,000,000, Group Personal Accident (main policy) with maximum coverage of S$600,000, Living Care (rider) with maximum coverage for S$350,000, Living Care Plus (rider) with maximum coverage for S$300,000/S$200,0001, Group Disability Income (rider) and Outpatient Medicare (rider).
1 S$300,000 if the application for coverage is made before age 56 years at next birthday or
$200,000 if the application for coverage is made at age 56 years old and above at next birthday
I would like to apply $1million coverage of Group Term Life (GTL) policy and I declare my pre-existing medical condition, will Aviva decline my application?
Aviva will accept the first $250,000 GTL application without underwriting. The first $250,000 GTL excludes pre-existing medical condition for 1 year. While Aviva will to the maximum extent possible, give favourable consideration to all applications, Aviva reserves the right to decline or postpone the application or offer a reduced sum assured for sums beyond the $250,000.
Do all the MINDEF & MHA Group Insurance Voluntary Scheme policies provide worldwide coverage?
Yes. All the MINDEF & MHA Group Insurance policies provide 24 hours worldwide coverage regardless of geographical location.
How long can MINDEF & MHA Group Term Life cover me?
Group Term Life will cover you till age next birthday (ANB) 70.
Does this insurance cover adopted children and/or stepchildren?
Yes, if the relationship between you and your adopted/stepchildren is legitimate and can be substantiated legally.
In the unfortunate event that I pass away before my insured spouse (dependant), can my spouse continue with the insurance coverage?
Yes. Your spouse has the option to continue with the insurance coverage until age 70 (ANB).
Can MINDEF & MHA members/affiliate members and their dependants be insured under different policies?
First, any person would have to sign up for the basic Group Term Life policy or/and Group Personal Accident policy, then he/she can sign up for any other group riders (i.e. Living Care, Living Care Plus, Disability Income and Outpatient Medicare).
Can my spouse and children apply for Living Care policy?
Yes, as a MINDEF & MHA Group Insurance Voluntary Scheme insured member/insured affiliate member, your dependants can apply for Supplementary Living Care insurance if they:
- have an existing Group Term Life or/and Group Personal Accident policy; or
- are applying for Group Term Life or/and Group Personal Accident policy with this rider.
Can I sign up for Group Disability Income insurance? What should I do if there's a change in my income?
Yes, you can sign up for the Group Disability Income insurance if you are employed and insured with the MINDEF & MHA Group Term Life or/and Group Personal Accident policy. You should contact Aviva Ltd to update the change in your occupation and/or income, and we will advise you the procedure accordingly. Otherwise, in the event of a claim and your occupation or current monthly salary are found to have differed from what was declared to Aviva Ltd, there could be delay or complications in the claims process.
I am a SAF NSman and a Home Team officer. Can I sign up both MINDEF Group Insurance and MHA Group Insurance?
No, you can't. MINDEF & MHA Group Insurance is under one contract. You can either choose MINDEF Group Insurance Scheme or MHA Group Insurance Scheme.
How do I make a claim?
In an unfortunate event that I pass away, how will the death benefit be paid out?
In the event of a Death Benefit, Aviva will pay the first $150,000 to the proper claimant. The balance of the Death Benefit will be paid according to the prevailing insurance laws and other relevant laws pertaining to estates of deceased. The Nomination of Beneficiary framework under the Insurance Act is not applicable for Group policy arrangement.
Can I claim for hospitalization expenses under Group Term Life policy?
Group Term Life pays in the event of Death, Total Permanent Disablement or Advance Payment Benefit but not for hospitalization expenses. However, this insurance does pay daily hospital cash benefit, for continuous hospital confinement starting from the 11th day of the hospitalization (up to a maximum of 30 days per hospital stay).
If I am insured with Group Term Life policy with Living Care policy, and I am diagnosed with one of the 37 critical illnesses, what will happen to my Group Term Life policy?
If you are diagnosed with one of the 37 critical illnesses and the claim is admitted and paid by Aviva Ltd, the sum assured of your Group Term Life policy will remain unchanged. You may continue with your Group Term Life policy at the prevailing monthly premium rate.
What shall I do if I do not have enough money in my bank account for the monthly GIRO deduction?
You may wish to visit our Customer Service Hall at 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 to pay your premium using cash/NETS/cheque before the last business day of the month.
How can I make my monthly premium payment if I have insufficient funds in my GIRO account and am currently not in Singapore?
You may wish to make arrangement with your family members in Singapore to make the payment. They can visit our Customer Service Hall at 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 to pay the premium using cash/NETS/cheque before the last business day of the month.
How do I change my GIRO bank account?
Please complete a new GIRO application form and mail the form back to Aviva Ltd. Please note that you should not terminate your existing GIRO bank account until your new GIRO bank account is approved.
If you have terminated your existing bank account, please submit a new GIRO application form together with 3 months' premium payment via cheque made payable to 'Aviva Ltd'. On the reverse side of the cheque, please indicate your Full name, NRIC number, contact number and "Payment for MINDEF & MHA Group Insurance".
However, if you are changing your GIRO bank account between POSB Bank and DBS Bank, you can simply do so by visiting any of the bank branches and the change can be effected immediately. The Bank will keep us informed of the change.
When will the premiums be deducted from my GIRO account?
GIRO deduction is done on the 10th of every month. If the deduction is unsuccessful, a 2nd attempt will be done on the 25th of the same month. If the deduction dates fall on a weekend or public holiday, the banks will bring it forward to an earlier working day.
What is the grace period for payment of premium?
The grace period begins on the premium due date and lasts for 45 days.
Is there a free look period?
Within fourteen (14) days upon receipt of the Policy, you may write to us to cancel the Policy. After we have received the original Policy for cancellation, we will refund the premium you paid (without interest) after deducting any expenses incurred in issuing the policy and any amount of claim that were paid out. If the policy is sent to you by post, you are considered to have received it seven (07) days after the postage.
Will I receive annual renewal notice?
There is no annual renewal notice or renewal letter. The policy year starts on 1 October of the current year to 30 September of the following year for MINDEF/SAF insured persons; 1 July of the current year to 30 June of the following year for MHA insured persons. Renewal is automatic every year, all the way up to age 70 (ANB). However, once a year you will receive a statement from Aviva Ltd on your insurance coverage and the premiums you had paid for the preceding year.
Insured members/insured affiliate members who pay premium yearly will receive a billing notification, one month before the expiry of the policy year.
Will a new policy document be given to me upon renewal every year?
No. Renewal is automatic up to age 70 (ANB). You will be issued with a simple insurance certificate when you first sign up. However, once a year at the close of every calendar year, Aviva Ltd will mail to all insured members/insured affiliate members a statement on their insurance coverage and the premiums they had paid in the preceding calendar year. The information in the statement is also to facilitate your filing for income tax reporting.
Once the premium is paid, does Aviva issue a receipt?
For insured persons who pay cash at the Customer Service Hall, a receipt will be issued. No receipt is offered for those who paid by cheque.
Can the premiums paid be used for tax relief?
Yes, the premiums can be used for income tax reporting, subject to the conditions set by the Inland Revenue Authority of Singapore (IRAS). Aviva will send a consolidated premium statement in the beginning of each calendar year to all insured members/insured affiliate members for premiums paid in the preceding calendar year, for tax reporting.
How do I terminate my/dependant's MINDEF & MHA Group Insurance policy?
To terminate, you will need to submit a signed hardcopy letter and make a photocopy of your NRIC for authentication. Please send it to: Aviva Ltd 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 (Attention: Group Life and Health Services). Alternatively, you can scan the documents and send via email to: MINDEF_insurance@aviva-asia.com (for MINDEF/SAF personnel) or MHA_insurance@aviva-asia.com (for MHA personnel). Upon receipt, we will terminate it on the last day of the month.
Can I come to your Customer Service Hall to sign up for MINDEF & MHA Group Insurance Voluntary Scheme or to terminate this insurance?
Yes, visit Aviva Ltd 4 Shenton Way #01-01 SGX Centre 2 Singapore 068807 and our staff will be able to assist you. Our opening hours are: Mondays to Fridays from 8.45am - 5.30pm. We are closed on Saturdays, Sundays and Public Holidays.
Why is it that I am not given an insurance policy document? I was only given an insurance certificate many years' back, but I have lost it. What is the procedure to request for duplicate policy or insurance certificate?
We do not issue policy document to the individuals. The insured members/insured affiliate members of the group scheme are issued with a simplified insurance certificate. This certificate is only issued once. However, if you would like to request for a reprint of the Certificate of Insurance, we will be able to assist. Change of Personal Particulars
I have formally changed my name. Please advise me how I can update my insurance record?
How do I update my address for my MINDEF & MHA Group Insurance?
I have received a lapse/termination letter, which stated that there were insufficient funds in my bank account last month. Can you advise me how I can reinstate my lapsed policy?
To reinstate the lapsed policy, you will need to complete and sign on a Reinstatement Form. Together with the Form, you will need to pay for the premiums in arrears plus an advance of 3 months' premium (by Cheque payment). Please submit a completed GIRO form, so that we can assist you to activate a new GIRO account for the purpose of monthly premium. However, if you have not changed or terminated your bank GIRO account, we can continue to deduct the premiums from your current GIRO account. However, reinstatement of your lapsed policy is subject to your current medical condition(s) which must be acceptable to our underwriters and you will be informed officially in writing.

Will my pre-existing condition be excluded?
Pre-existing condition(s) if not declared when he/she first applied or subsequently apply to upgrade his/her coverage will not be honored in the event of a claim. Pre-existing condition(s) will be subjected to underwriting and you will be communicated in writing of such underwriting decisions.
How long will it take to approve my application for MINDEF & MHA Group Insurance policy and commence coverage?
Your coverage will commence within 5 working days upon receipt of an application if the health declaration is acceptable by the underwriters. However, if there is a medical issue and further medical evidence(s) is/are required, it would take longer, depending on when the applicant can furnish the evidence to support his application.
Nomination of Beneficiaries
Can I make nomination of beneficiaries for MINDEF & MHA Group Insurance Voluntary Scheme?
The Nomination of Beneficiaries framework is not applicable for group insurance scheme. The main policyholder, that is, MINDEF and MHA, had directed Aviva to pay according to the prevailing laws of Singapore. That is to say, in the event of a Death Claim, the law provides for Aviva to pay to the immediate next-of-kin (NOK) or proper claimant up to S$150,000. The payout of the balance of the insurance compensation has to be decided by a Court.
If the deceased had written a Will before his/her death, the family has to engage a lawyer to apply to the Court for a Probate, which may take up to 6 months.
If the deceased did not have a Will before his/her death, he/she would be considered to have died intestate. In such an event the family also has to engage a lawyer to apply to the Court for a Letter of Administration, which may take up to 3 years to resolve.
