Who is eligible to apply for the DPI plans?
Any resident in Singapore (including foreigners who are holding a valid Employment Pass, Work Pass, Dependant Pass or Student Pass) who is between 19 to 65 years old (age next birthday).
Do I need to submit any medical reports or undergo any medical check-ups?
Yes.
How do I know if my application has been successfully completed?
We'll send you a confirmation email and mail you the policy documents. If there are issues or further details from you are needed, we will call or email you to let you know.
How can I make a claim?
You'll need to complete a claim form and return it to us. There are four ways to do so:

- Call our customer service hotline at +65 6827 9933
- Email cs_life@aviva-asia.com to request for one
- Visit our Customer Service Hall during office hours to obtain a printed form
