Everyone has unique needs at different stages of their recovery journey. Aviva's Patient Support Programme runs for up to 12 months depending on your need for support. Each plan is carefully customised and may include a combination or all of the following:


A personalised care plan

Your dedicated care team will access all your needs such as gathering the necessary information including your medical data and contact details of your healthcare providers to help you prepare for your post-treatment healthcare journey. 


Appointment coordination

Your care coordinator will help you plan a calendar of all your upcoming medical appointments including your rehabilitation therapy sessions and send you timely reminders before each appointment.


Get holistic support

In consultation with your healthcare providers, both you and your caregivers will receive information, guidance and training on preventive care, nutrition, stress management and assistance to seek financial aid where necessary.


Road to recovery

As you progress in your recovery journey, your care plan will be monitored and adjusted to ensure you receive optimal support that will promote your recovery and enhance your quality of life. 

An example of how it works

Patient's profile: A male patient in his mid-40s was diagnosed with bone cancer on his left leg. He suffers from chronic pain, a limping gait, a gastric ulcer and has a history of falls.  

The objective: To prevent unnecessary hospitalisation due to his falls and a gastric ulcer.
