Terms and conditions for Mount Alvernia birthday perks campaign



1. Only qualifying customers are eligible for the birthday perks under this Campaign (the “Gift”). To be a “qualifying customer”, the customer must:

(a) be a customer of Mount Alvernia Hospital and received the birthday card (for Mount Alvernia babies);
(b) complete an e-form and agree to be contacted by an Aviva Relationship Consultant for information, offers and promotions about Aviva products and services; and
(c) complete a portfolio review with the Aviva Relationship Consultant.

2. Each qualifying customer can redeem only one (1) free Gift under this Campaign, limited to one (1) redemption per birthday card and per residential address.

3. The Gifts are supplied by various vendors (the “Suppliers”) and are redeemable directly from the respective Supplier(s). The Suppliers’ terms and conditions apply.

4. This Campaign is subject to availability of the Gift and limited to the first 200 redemptions per calendar month and is not valid with other offers, bundles or promotions unless stated otherwise.

5. Aviva reserves the right to amend these terms and conditions (including the contents and value of the Gift) or withdraw or discontinue this Campaign at any time without notice or liability.

6. The Gift is not transferable and is not exchangeable for cash, credit or any other item in part or in whole and is not replaceable, if expired, lost or stolen. Aviva reserves the right to replace the Gift with items of similar or other value at its absolute discretion, at any time without prior notice.

7. Aviva makes no representation or warranty whatsoever as to the quality or fitness for purpose or any other implied terms or conditions with respect to the Gift. Any dispute about the Gift must be resolved directly with the suppliers of the Gift. Aviva is not liable for any injury, claim, loss or damage whatsoever arising in connection with the Gift.

8. By participating in this Campaign, the customer consents under the Personal Data Protection Act (Cap 26 of 2012) (“Act”) to the collection, use and disclosure of the customer’s personal data by/to Aviva and such other third party for the purpose of this Campaign, including the third parties administering the redemption, or any third parties that Aviva may, in its absolute discretion, consider appropriate or necessary in connection with this Campaign. The customer confirms that he/she has read and agree to be bound by the terms of the Aviva Data Protection Policy, as may be amended, supplemented and/or substituted by Aviva from time to time, a copy of which can be found on www.aviva.com.sg/pdpa.html.

9. By participating in this Campaign, the Customer accepts that Aviva’s decision on all matters relating to the Campaign is final and binding on the customer. If there is any inconsistency between these terms and conditions and any brochure, marketing or promotional material relating to the Campaign, these terms and conditions will prevail.

10. A person who does not participate in the Campaign shall have no right under the Contracts (Rights of Third Parties) Act (Cap.53B) to enforce any of these terms and conditions.

11. The terms of the Campaign will be governed by and construed in accordance with the laws of the Republic of Singapore and the Customer agrees to submit to the jurisdiction of the courts of the Republic of Singapore.
